PCS Checklist
(Before Leaving Current School, Please Gather These Documents To Bring To Japan)

  • Immunization Record
  • Passport or Birth Certificate for kindergarten
  • Child's Social Security Number
  • Copy/Official Transcript
  • Copy of withdrawal grade with grading system from previous school
  • Copy of IEP (Individualized Educational Plan) if applicable
  • Copy of Standardized Test Scores, if applicable
  • Copy of Current Schedule (for high school students)
  • Copy of School Profile (for high school students)
  • Copy of Student Handbook (helpful for new school to use in interpreting transcripts)
  • Course descriptions (if not included in handbook)
  • Student resume or list of extra curricular activities (for high school students)
